Client Agreement for Consultation Using the Interactive Query System (IQS)

By proceeding with my appointment, I acknowledge and agree to the following terms and conditions:

1. Nature of Services and Medical Disclaimer

• I understand that the services provided by dentro de si Holistic Health Discovery Center are not a substitute for the care, diagnosis, or treatment provided by a licensed medical professional.

• These services are intended strictly to support, not replace, conventional medical care.

• The purpose of my consultation is to assist me, the client, in exploring ways to support my own health, as well as that of my dependent children and/or pets, through non-medical holistic approaches.

• I understand that I am seeking assistance for my own well-being and not for diagnosis or treatment of a medical condition.

2. Understanding the Interactive Query System (IQS)

• I understand that dentro de si Holistic Health Discovery Center utilize Interactive Bio-Feedback (IBF) technology, also referred to as the Interactive Query System (IQS), which is based on empirical observations.

• Any health-related information obtained through this technology reflects my body’s energetic responses and does not constitute a medical diagnosis.

• If I wish to receive a diagnosis or medical opinion regarding my condition, I will consult a licensed medical provider.

3. Client Responsibility and Recommendations

• I acknowledge that any suggestions, recommendations, or custom remedies made by the staff or associates of dentro de si Holistic Health Discovery Center are intended as supportive information.

• I understand that the HeartSong Collection™ drops and the InnerVoice™ Oil Blend are supportive in nature, not medical.

• It is my responsibility to evaluate these suggestions and determine whether to implement them, taking into account all available information and consulting with appropriate medical professionals as needed.

4. Fees and Confidentiality

• I acknowledge that the services provided are not medical in nature and are not eligible for coverage under health insurance plans.

• I understand and agree that the recording of any Session, in any format and on any device, is strictly prohibited.

By continuing with my scheduled appointment, I confirm that I have read, understood, and agreed to the terms outlined above.
